This Service Agreement ("Agreement") is made and entered into on 03/13/2025 by and between:
Pup-tism ("Service Provider"), located at [Static Pup_tism Address], and:

Scope of Services:
Pup-tism agrees to provide ceremonial services for the Client's dog(s) at the Client's home. These services will last for a duration of two (2) hours.

Terms and Conditions:
Payment: Payment for services is due in full at the time of booking.
Service Fee: {Select Package:7} , {Total:9}
Cancellation Fee: A non-refundable $100 cancellation fee will apply for any cancellations by the Client.

Safety Requirements:
All dogs must be up-to-date with vaccinations, including rabies, and documentation must be provided upon request.
Dogs must be friendly or securely muzzled if necessary.
If the Service Provider feels unsafe, the ceremony may be conducted from a safe distance or location as determined by the Service Provider.

Bonded Services:
Pup-tism is fully bonded and ensures responsible and professional service at all times.

Client's Responsibilities:
The Client must ensure that the Service Provider has access to a clean and safe environment for conducting the ceremony.
The Client agrees to disclose any behavioral concerns or special instructions related to the dog(s) in advance.

Liability:
Pup-tism will not be held liable for any injuries or accidents caused by the dog(s) during the service if proper precautions (e.g., muzzle, leash) are not adhered to by the Client.
Pup-tism is not responsible for damages caused by unforeseeable incidents during the service.

Rescheduling and Refunds:
If the Client reschedules the appointment at least 48 hours in advance, the full payment can be applied toward the new date.
Cancellations made less than 48 hours before the scheduled service are subject to the $100 cancellation fee, and the remaining payment will not be refunded.

Termination of Services:
Pup-tism reserves the right to terminate the service at any time if the environment is deemed unsafe or if the dog poses a threat to the Service Provider or others.
